Description
This application is a hospital post-discharge Patient Engagement Information Service that combines real-time patient communication with nurses following discharge from the hospital. Patients are registered in the application before leaving the hospital. The following day after discharge, the phone application will prompt patients to answer 4 to 8 questions about their recovery each day. Patients will be asked about their medications, how they are sleeping, eating and feeling each day. Responses are immediately sent back to a team of nurses at the hospital, who receive alerts when patients report problems in their recovery. Nurses from the hospital may then contact patients to assist them in their recovery.
